I once read a fascinating book, Unnatural Death, by (if memory serves) Dr Helpurn who was the first forensic pathologist for New York where he touched on both autoerotic deaths and (misadventure) deaths that occured during riskier BDSM games, often, the tragedy stemmed from rank stupidity - leaving a man balanced in a choking position and leaving him for a while - he strangled!
